What Is Support At Home
The Support at Home Program is a key part of the government’s aged care reform, bringing together several existing initiatives — such as Home Care Packages (HCP), the Commonwealth Home Support Program (CHSP), and Short-Term Restorative Care — into one simplified and more coordinated system.
Its purpose is to provide older people with greater choice, flexibility, and control over their care, ensuring access to the right level of support when it’s needed most. Under the new model, funding will be distributed across eight care levels (an expansion from the current four under HCP), along with three short-term care pathways, making it easier to align services and funding with each person’s individual needs.
How Support At Home Works

You’ll start with an aged care assessment arranged through My Aged Care to understand your support needs and determine your funding eligibility.
2. Budget Allocation
Instead of a fixed package level, you’ll receive an individualised budget that reflects your personal care requirements.
3. Care Plan
Together, we’ll design a tailored care plan that supports your lifestyle, goals, and independence.
4. Service Delivery
Your Mycare Bridge team will coordinate your in-home services, review your progress, and adjust your support as your needs evolve.

Who is eligible for the Support at Home Program?
The Support at Home Program is designed for older people who wish to remain living independently in their own homes. You may be eligible if you are 65 or over, or 50 or over if you are an Aboriginal or Torres Strait Islander person, and need help with daily activities such as cleaning, personal care, transport, or meal preparation. Eligibility is determined through an assessment arranged by My Aged Care, which helps identify the level of support that best suits your needs.
What should I expect during a Support at Home assessment?
During your assessment, an assessor will visit you at home to learn more about your health, lifestyle, and the kind of help you may need with everyday activities. This conversation helps determine which services will best support your independence and wellbeing. The assessor may also discuss local service providers and can send referrals to your preferred provider. They will explain any potential costs and share information on where to find further guidance. You’ll have the opportunity to ask questions or raise any personal or cultural considerations, and you’re welcome to have a family member or friend present for support.
